Hello! I hope you and your family are doing well and you are staying out of all these crazy winter storms.

We had our 3 boy on 12/31 so I needed to file a claim – can you remind me how to do this?
I also saw something in one of my folders (looking for the above info) about a wellness claim. I’m not sure I remember
doing this the last few years – is there a deadline to get these done – as in a use it or lose it? Do I have the option to go
back and submit for this (assuming my policy offers one of these?)
