Further investigation revealed that the gearbox motor coupler had slid up on the shaft and separated from the grinder coupler. To prevent this from happening again, the couplers were removed, taken to a machine shop, and equipped with set screws. Additionally,we discovered that the high-level alarm system had not been triggered.Upon inspection,it was found that the float wires going to the PLC were not properly connected. An electrician rectified this by installing the wires and testing the floats to ensure they would alarm as designed. The IPDES report line on July 16 at 7:22 a.m.,repotting an estimated spill of 5 to 10 thousand gallons into Rock Creek. Samples were collected from Rock Creek and sent to the lab to be analyzed.
